Just choose six activities that aren't part of your regular routine (ask about our Workouts of the Week for inspiration), then put your choices down on the left of your personal activity card - ask a member of the fitness team in your club to get hold of one.

The aim of the game is to get through your six workouts in six weeks.
We'll stamp your card whenever you take part in a session. Then, when you've collected the full set, pop your completed form into our drop box.
Three lucky winners will be drawn at random to win a month's free membership.
